How it works
We offer an easy 3 step application process
- Initial Evaluation Schedule your initial evaluation appointment today! You can book online here. Once scheduled, you will need to fill out the intake form here, prior to your appointment. At your initial evaluation the doctor will review your health conditions, discuss your treatment options, and issue your certification and medication orders.
- Register With State of Florida We will enter you into the Florida State Registry during the initial face to face appointment. The state will then email you a login. In order to receive the required ID card, you must submit a completed application to the state along with the required $75 fee here. The registry is connected to FLHSMV, which means your state ID/driver’s license can be automatically added to your application. We will be happy to help you through this process.

1 How old do I have to be to apply for a Medical Card in Florida?
You must be at least 18 years of age in order to apply for a Medical Card in Florida. If you are under 18, you must have your parent or guardian present during the examination.

2 Can I grow my own product?
Patients are currently not allowed to grow.

3 What is the prescription amount?
Florida State law allows you a 70 day supply of medicine at a time.

4 Will I still be charged if I do not qualify for a medical card?
If you don’t receive a recommendation, you would be issued a full refund. No Questions Asked!

5 How long does the entire process take?
The examination itself takes approximately 15 minutes. After payment is submitted to the state, it then takes 5 business days for the payment to process. Approximately 10-17 business days later, you will receive a second email from the state. You can use this as a temporary card until you receive your official Medical Card in the mail.

6 How often do I need to see the doctor?
Patients must be seen for an in-office visit once every 30 weeks from your initial examination.
